EDIT : List of some tweaks which occured:

Level 3. Place the InstaKill somewhere else.


Level 7. Wrong ID.


Also level 7. This is look on layer 6. This doesn't occur in any other Madmonks level, though. Similar thing can be seen in Level 5 and 6 (Iceland) with igloos.

There of course might be more.
