...you know, that's a good question. I do have that CD case, but there's been a sticker over that particular screenshot for the last couple decades, and I don't think it's coming off. The mode 7 part at least is from BONUSY.000.

As for that floppy... the 1993 date is weird, but maybe it's referring to some sort of wrapper software the WIZ company put in (note that you apparently type WIZ, not JAZZ, to run it). Maybe there are other pictures out there of other WIZ games with the same copyright notice?
